Have you tried running the command manually?
/oracle/<SID>/112/bin/srvctl add database -d <SID> -o /oracle/<SID>/112 -p +<SID>_DATA/<SID>/spfile<SID>.ora -a <SID>_DATA
What is the output of the command?
